Cooperative Rural Banks

i. Formal Letter of Application (format from Nationlink)

ii. Letter of Endorsement from Affiliate Network (c/o Nationlink)

iii. Bank/ Coop Profile

  • Brief history
  • Copy of SEC Registration/ DTI Registration
  • Copy of CDA Registration
  • Copy of Latest Business/ Mayor Permit
  • Location - complete address/contact no. of head office and indicate if premises is owned or rented
  • Ownership/ Capital structure - name, % of ownership, no of shares, type of shares and amount subscribed and paid for
  • List of Officers and Directors
  • Curriculum Vitae of the ff officers: Chairman of the Board, Board of Directors, President, Treasurer, COO or General, Manager and majority, shareholders (if not one of the officers).
  • List of Affiliates and/or Subsidiaries
  • Products and Services - deposits, loans, etc.
  • Branch Network - branch name, location and contact number
  • Provide site map or vicinity map

iv. Audited Financial Statements for the past three (3) years and interim Financial Statements of Current Year as applicable


v. Certification of the PESO-COOP Ratings based on the latest Audited FS and signed by local CDA Officer. (Benchmarks: PESO COOP Weighted Average Rating should be 80% and above)


vi. Description of IT Structure - to include ATM Switch, CASA System


vii. ATM Machine Deployment Plan for the first three years, signed by the institution’s President/CEO (indicate number of ATMs to deploy, location and time table)

viii. ATM card projection-number of cardsexpected to issue on the first three years with breakdown per year, signed by the institution’s President/ CEO


ix. Letter of Authorization for Dun & Bradstreet to conduct verification of certain documents with BSP


x. ATM Card Design (For Nationlink Approval) 

i. Formal letter of Application from Bank to Bancnet (format from Nationlink)

ii. Letter of Endorsement from Affiliate Network (c/o Nationlink)

iii. Bank Profile

  • Brief history of the bank - indicate year incorporatd
  • Copy of SEC Registration/ DTI Registration
  • Copy of BSP Certificate of Authority
  • Copy of Latest Business/ Mayor Permit
  • Location - complete address/contact no. of head office and indicate if premises is owned or rented
  • Ownership/ Capital structure - name, % of ownership, no of shares, type of shares and amount subscribed and paid for
  • List of Officers and Directors
  • Curriculum Vitae/Biographical Data of the following: Board of Directors, President, Corporate Secretary, Treasurer, COO or General Manager.
  • List of Affiliates and/or Subsidiaries
  • Products and Services - deposits, loans, etc.
  • Branch Network - branch name, location and contact number
  • Provide site map or vicinity map

iv. Audited Financial Statements for the past three (3) years and interim Financial Statement


v.Submission of “No Objection Letter from the Bangko Sentral ng Pilipinas (BSP).


vi. Design of the Bank’s ATM Card for Nationlink approval
